How do I enter or change my pharmacy information?

To enter your pharmacy information, you can select "Appointments" from the Maven side menu, then select the individual appointment to view. There, you'll see a Pharmacy Information option where you can fill out all relevant details.

Here's a quick trick: if you search by the zip code of your pharmacy (instead of your home zip code) as the zip code and the pharmacy store number as the pharmacy name, you'll see your pharmacy and be able to select it for your provider! For example, if your pharmacy is Store #12345, we recommend entering "12345" in your pharmacy name spot to search.
