Maven providers do not typically complete paperwork such as disability, medical leave, or other formal documentation. These requests are usually best handled by an in-person provider who can perform a physical evaluation if needed.
In some cases, a Maven provider may choose to support paperwork requests if you have an established relationship. This support is not guaranteed and is always at the provider’s discretion.
When you may be eligible for support
To be considered for help with paperwork, all three of the following guidelines must be met:
- You’ve established care with a Maven provider who is licensed in your state
- You’ve had at least three (3) appointments with the same provider
- You’re unable to find an appropriate in-person provider to complete your paperwork
How to request help
If you meet the guidelines above, message your provider directly to ask whether they’re able to assist. They’ll review your request and let you know if they’re comfortable supporting you.
If your provider can’t complete the paperwork
Even if your provider isn’t able to complete paperwork, they can still support you in other ways, including:
- Talking through next steps or documentation you may need
- Helping clarify requirements
- Sharing referrals to in-person providers who may be able to assist
Important note
Paperwork support is based on a provider’s clinical judgment and is not guaranteed. In many cases, documentation may require an in-person visit or a provider with specific licensing, so a Maven provider may not be able to complete or approve every request.
