Maybe! It depends on the exact forms and whether an in-person or long-standing relationship would be necessary for a provider to complete the requested information. We recommend that you message a provider and/or complete our Support Form before booking an appointment to confirm that a specific provider would be a good fit to discuss this request.

Importantly, even if you confirm that a provider is a good fit for an evaluation, it is always up to the discretion of the individual provider after a video consultation and full evaluation whether or not a note/form is appropriate.
