Yes, in certain circumstances, your Maven provider can help support you with disability and medical leave paperwork. To be eligible for this support these three guidelines must be met:
- You must establish care with a provider on Maven who is licensed in your state.
- You must have at least three (3) appointments with this same Maven provider.
- And you must be unable to find an appropriate in-person care provider to complete your disability or medical leave paperwork.
If you meet the guidelines above, please contact your Maven provider directly to request assistance. They will determine if they’re comfortable supporting you in this.
If your provider is unable to complete your paperwork directly, they can still provide valuable support. In addition, Maven can put together a list of highly rated, in-person, and in-network providers outside of Maven who may be able to assist you.
Important note: Completion of disability or medical leave paperwork is at the discretion of the provider and based on their clinical judgment. In some cases, completing this paperwork may require an in-person visit or a provider with specific licensing. As a result, we cannot guarantee that a Maven Clinic provider will be able to complete or approve the paperwork you are requesting.
