In some situations, a Maven provider may be able to support you with disability or medical leave paperwork. This support depends on a few important requirements and is always at the provider’s discretion.
When you may be eligible for support
To be considered for help with paperwork, all three of the following guidelines must be met:
- You’ve established care with a Maven provider who is licensed in your state.
- You’ve had at least three (3) appointments with the same Maven provider.
- You’re unable to find an appropriate in-person provider to complete your disability or medical leave paperwork.
How to request help
If you meet the guidelines above, reach out to your Maven provider directly to ask whether they’re able to assist. Your provider will review your request and let you know if they’re comfortable supporting you with the paperwork.
If your provider can’t complete the paperwork
Even if your Maven provider isn’t able to complete your paperwork themselves, they can still support you in other ways. This may include:
- Discussing next steps or documentation you may need
- Helping clarify requirements
- Sharing a list of highly rated, in-person, and in-network providers outside of Maven who may be able to assist
Important note
Completion of disability or medical leave paperwork is based on the provider’s clinical judgment and is not guaranteed. In some cases, paperwork may require an in-person visit or a provider with specific licensing. Because of this, a Maven provider may not be able to complete or approve every request.
