If your partner or parent’s employer or health plan offers Maven as a fully sponsored benefit, you’ll need to enroll in a Maven sponsored program to access your benefits. Here’s how:

Create a Maven account
- If you don’t already have one, you can create a Maven account by downloading the Maven Clinic app or visiting mavenclinic.com/register.
- Also, we recommend reading our “How do I set up my Maven Account” article.
- Check your enrollment status
- Go to the "Me" tab (on mobile) or your initials (on the web).
- If you see a "Program" tab with your program name listed, you're enrolled!

Enroll in your sponsored program
If you’re not currently enrolled, click “Sign me up” on your dashboard and follow the instructions to verify your eligibility.
4. Enter your partner/parents organization, you will be asked to verify the following information:
- Partner/Parent’s Date of Birth
- Partner/Parent’s work email address, if they are sponsored through their employer.
- If they don't have a work email, you will be asked to fill out their first & last name and DOB. Also, you may be asked for their work state.
- Subscriber ID
- You can find this on the sponsored individual’s health insurance card (it may be called “Member ID” or “ID)
5. Once we’ve verified your information, we’ll ask you to select the Maven program that best fits your needs right now (you can always change this later).
6. After selecting your program, we’ll ask you a few health questions to create a Maven Care Team and journey that’s personalized to you.
