If your employer/health plan or your partner or parent’s employer/health plan offers Maven as a fully sponsored benefit, you’ll need to enroll in a Maven sponsored program to access your benefits.
If you’re having trouble enrolling in your Maven benefits, try these tips:
Enter the date of birth using the format MM/DD/YYYY.
The format MM/DD/YY is unique to the United States (but sometimes used in Canada too) which can create some confusion. When enrolling in your Maven sponsored benefits, please enter your date of birth using the format MM/DD/YYYY (e.g. January 15, 1980 becomes 01/15/1980).
Enter name as it appears in the employer's or health plan's records.
When asked to enter your name, the employee’s name, or the sponsored individual's name, we recommend entering names exactly as they appear in employer or health plan records. If you’re unsure, verify with the employer or health plan provider how they have the name listed on their eligibility file.
Also, these tips can help you when entering your name:
- Avoid using nicknames or abbreviated versions of names. (e.g. if you go by Kate but your official name is Katherine, we recommend entering Katherine when trying to enroll).
- If your last name includes a hyphen it can help to remove the hyphen.
- If your last name has changed since employment or enrollment in your health plan, try entering your original last name.
Please keep these timelines in mind when enrolling:
- If your employer or health plan has recently announced adding Maven benefits, you will not be able to enroll in your sponsored benefits until your organization has officially launched with Maven.
- If you, your partner, or your parents are new hires, it may take a few weeks to appear on the employer’s eligibility file.
- If your sponsored benefits are through your health plan, your name will not appear on their eligibility file until your health plan coverage starts.
- If the employer requires enrollment in a company-sponsored health plan to access Maven, your name might not appear on their eligibility file until your coverage starts.
